At The MENTOR Network, we know that parents and family members want to keep their loved ones close and help them live life to the fullest—especially when they are transitioning from hospitals or other institutional settings into the community.
Our Community-based Residences—sometimes referred to as group homes—offer adults with intellectual and developmental disabilities the opportunity to live as independently as possible in the communities of their choice. Nestled in residential neighborhoods across the country, our Community-based Residences offer 24/7 staff support, expert clinical services, and opportunities for individual growth.

The MENTOR Network is a nationally recognized leader in developing community-based residences that support individuals with a range of abilities. The Network’s first home—where we continue to successfully serve individuals through our partner, REM Minnesota—opened nearly 50 years ago. Since then we have continued to strengthen our residential programs and build an extensive network of resources. This allows us to open and prepare new homes that meet the needs of adults in communities across the country.

Our residential options range from Intermediate Care Facilities (ICFs) for individuals who require intensive medical care and nursing services to smaller group homes and apartments for those who are able to live more independently.
Individualized Service Planning
When someone is ready to move into one of our homes, we take the time to understand their needs and interests. Our homes are staffed with Direct Support Professionals (DSPs), nurses and other human services professionals who develop Individualized Service Plans (ISPs) to help each individual reach new milestones.
We offer personalized services, including:

- 24/7 on-call support

- Management of daily activities
- Structured activities
- Life skills development
We take pride in the quality of our programs. As with all of our services, our community-based homes adhere to state and Network requirements and are carefully monitored by our continuous quality improvement team to ensure the health, safety and happiness of each person we support.
