At The MENTOR Network, we understand that the process of selecting services and making informed choices to meet unique goals can be difficult. That’s why we offer Case Management and individualized service planning for all of the adults we serve through our continuum of supports for individuals with intellectual and developmental disabilities.
We help individuals and families assess their needs and coordinate services. Our staff also advocate on behalf of the individuals we serve, as well as their families, to ensure that their voices are heard in selecting services and other community supports.
Individualized Services and Supports
We use a personalized approach and create Individualized Service Plans (ISP) for each person we serve in collaboration with all members of their circle of support. Each adult receives a formal assessment by a professionally trained coordinator. The assessment is used to develop an ISP, which includes the individual’s lifestyle preferences within a full range of residential, work, social and other community-based services and supports. The plan is assessed and altered on an ongoing basis, according to the individual’s needs.
The MENTOR Network also offers crisis intervention services, primarily in the form of short-term emergency placements, together with intensive behavioral therapy, relief support for families, or other services aimed at stabilizing an individual’s placement.
